1. Professional Services Basic services of funeral director, staff and overhead Our fee for the basic services of funeral director and staff includes, but is not limited to, staff responding to the initial request for service; arrangements, conference with family and responsible parties; preparation and filing of necessary authorizations and permits; recording vital statistics; preparation of obituary notices; shelter of remains; coordination with those providing other portions of the funeral, e.g., cemetery, crematory, vault companies, clergy, musicians, and others required. Also included in this charge are overhead expenses relative to our facilities such as: professional licensing, legal and accounting fees, insurance, building and utility expense, parking lot and grounds maintenance, taxes, equipment, furnishings, inventory cost, record keeping, and secretarial and administrative expenses. The fee for our basic services and overhead will be added to the total cost of the funeral arrangements you select. A portion of this fee is already included in our charges for a direct cremation, immediate burial, forwarding or receiving of remains. 2. Traditional Professional Service with Cremation Following Service Basic Services of funeral director, staff and overhead 3. Cremation with a Memorial Service Service at our chapel or a church 4. Direct Cremation Our charge for direct cremation includes: basic fee of funeral director and staff, a proportionate share of overhead costs, bringing remains into our care from place of death, local transportation of remains to crematory, crematory fee. It does not include: use of facilities for visitation or service (public or private) prior to or after the cremation, items of merchandise, paid newspaper notices, cremation permits, death certificates, or any other cash advanced item. If you want to arrange a direct cremation, you can use an alternative container. Alternative containers encase the body and can be made of materials like fiberboard or composition materials (with or without an outside covering). The alternative containers we offer are hardboard composite or fiberboard boxes. 5. Immediate Burial Our charge for immediate burial includes: basic services of funeral director and staff, bringing remains into our care from place of death, filing necessary authorizations, local transportation to cemetery, a proportionate share of overhead. It does not include: the use of our facilities for visitation or ceremony (public or private) prior to burial, items of merchandise, paid notices, or any other cash advanced items. 6. Forwarding of Remains to Another Funeral Home This charge includes: basic services of funeral director and staff, bringing remains into our care from place of death, embalming, filing of necessary documents, transportation to common carrier, a proportionate share of overhead. It does not include: use of staff or facilities for visitation, rites, or ceremony (public or private) prior to shipment, additional preparations, items of merchandise, shipment charges, newspaper charges, or other cash advanced items. 7. Receiving Remains from Another Funeral Home This charge includes: basic services of funeral director and staff, local transportation of remains from common carrier to funeral home, transportation to cemetery, a proportionate share of overhead. It does not include: use of facilities for visitation or funeral services, items of merchandise, or cash advanced items. 8. Cash Advanced Items (Those items or services provided by a third party but included on our statement) The funeral home assumes NO responsibility for cash advanced items not listed on our statement of goods/services selected. |
